We have prepared three ACA questionnaires for you to use in assisting clients. Included in this package is a Worsheet for the Marketplace Application (designed for use by a tax professional), a Medicare and Medicaid ACA questionnaire, and a questionnaire for clients without Medicare/Medicaid coverage.
