How to Obtain a Copy of the 501(c)(3) Determination or Make Changes

Affirmation Letter

I talked to a great assistor this morning and got the answer to a stakeholder's question. How do I get a new exemption letter with our new name and address?  When a tax exempt organization needs a new exemption letter, it is called an "Affirmation Letter." You can call, write or fax to get the letter. In this situation it was a little more complicated since they also needed to change the name and the address. A call alone will not take care of all three things.

First, is the organization incorporated?  If so, they will need a copy of the certified amended articles of incorporation showing the name change.  They would send that along with a cover letter requesting the name change, the address change, and an affirmation letter with the new name and address. This request must be signed by a corporate officer with their title.  This can be faxed or mailed.
